At Project Homeless Connect, we know that every person’s path to housing looks different. Some people need a quick connection to the right resource after leaving a shelter or hospital. Others need longer-term support as they work toward stability. Our housing team walks alongside individuals at every stage of that journey.
Thanks in part to Metro Supportive Housing Services (SHS) funding, Project Homeless Connect is able to provide several housing-focused programs designed to meet people where they are and help them move toward stable housing.
Three housing programs are operating within Project Homeless Connect.
Housing Liaisons
Our Housing Liaisons provide immediate, short-term support for individuals experiencing homelessness, including those leaving shelters, hospitals, or other crisis situations. We help connect people to housing resources, community services, transportation, identification assistance, and other critical supports while helping them navigate next steps.
Rapid Rehousing
Rapid Rehousing helps individuals and families move into stable housing with medium-term support, typically lasting less than two years. Participants receive housing navigation, case management, and temporary rental assistance while working toward long-term stability and self-sufficiency.
Housing Case Management Services
Housing Case Management Services (HCMS), supported through Washington County’s Regional Long-Term Rent Assistance (RLRA) program, pairs individuals experiencing homelessness with a dedicated case manager who helps them secure housing and connect to supportive services throughout the community.
*Housing vouchers and rental assistance resources are limited, and waitlists are often long with no guarantee of placement. PHC works within a larger countywide housing system, and referrals to our housing caseloads come through Community Connect (503-640-3263). Individuals can complete a Phase 1 housing assessment by calling Community Connect or coming to our our Access Center.

A Father’s Determination
After experiencing a sudden medical crisis that led to houselessness, “John” came to our Access Center focused on one goal: rebuilding stability so he could reunite with his infant son.
Through determination, consistent engagement, and support from Project Homeless Connect, John secured safe and stable housing and is now preparing for reunification with his son. His journey reflects the impact of support, resilience, and the opportunity to rebuild.
