Congregate and non-congregate shelters that protect people from the outside elements and offer greater stability.

Housing
Multiple programs that provide short-, medium-, and long-term housing assistance with case management.

Our team identifies and engages with individuals living in unsheltered locations, connecting them to resources to address immediate needs and find new pathways.


Access Center
A place of refuge, belonging, and resourcing; where people can receive warm meals, hot showers, laundry services, ID acquisition assistance, peer support, and connections to various resources, including shelters and housing programs.
Shelter
PHC manages two shelter models:
1. Year-round motel-style shelters.
2. Temporary congregate shelters are activated during severe weather conditions.


Housing
Three housing programs are operating within Project Homeless Connect: Housing Liaison, Rapid Rehousing Case Management, and RLRA Case Management.

Our team of peer support specialists and certified recovery mentors are dedicated to assisting individuals navigating their mental health and addiction recovery journeys.
