Our multi-pronged approach built upon trusting relationships includes our street outreach team, access center, shelter services, housing resources with case management, and one day events.

Outreach

Connecting individuals living in unsheltered locations to the homelessness service system, offering compassionate engagement and support in navigating services that promote wellness and stability.

Access Center

A place of refuge and connection where people can receive meals, referrals, and supportive guidance that fosters emotional well-being and next steps toward stability.

Shelter

An adult shelter with congregate and non-congregate beds that provides safety, stability, and supportive services that promote wellness and personal well-being.

Housing

Multiple programs that provide short-, medium-, and long-term housing assistance with case management.

Outreach

Our team identifies and engages with individuals living in unsheltered locations, connecting them to resources to address immediate needs and find new pathways. 

Access Center

A place of refuge, belonging, and resourcing; where people can receive warm meals, hot showers, laundry services, ID acquisition assistance, peer support, and connections to various resources, including shelters and housing programs.

Shelter

PHC operates two shelter models:
1. The new Hillsboro year-round shelter
2. Incelement weather shelters

Housing

Three housing programs are operating within Project Homeless Connect: Housing Liaison, Rapid Rehousing Case Management, and RLRA Case Management.

Our team of peer support specialists and certified recovery mentors are dedicated to assisting individuals navigating their mental health and addiction recovery journeys.

one day events

One-stop-shop events that provide extensive, critical services and resources for individuals and families experiencing homelessness or housing instability in Washington County, Oregon.
